Motor control on carriage

Last year I bought a "Gearhead" 13x40 lathe that is about 25 years old. It's very similar to an Enco 13 x 40 110-1351/510-2585 and Grizzly 4002. I really enjoy getting out to my shop and using it.

On the headstock there are an inch/run switch, reverse, stop and forward buttons. The carriage mounted motor control system is completely missing (no switches or mount on headstock, no lower shaft, no lever or mount on the carriage and an empty hole in the shaft block at the tailstock end) but I suspect it would be really handy and help keep me out of the way of flying chips, so I'm considering trying to put one together.

Is the carriage mounted control spring loaded, so that it returns to the center position after closing either the forward or reverse microswitch?
Is forward up or down?
Can the carriage lever also be used to stop the lathe?
Does the shaft have a keyway or flat?
Ideas about how to make a shaft, if I can't buy one? I don't have a mill....yet.

Thanks for your input.
