That looks very similar to the ratchet used on rolltite trailers for tensioning the tarp system

It looks to be a torque multiplier to me but the one I used years ago didnt have the bit adapter on the output end.

Most torque multiplier have a sun gear at the head. This appears to be an old ratchet wrench. Is the part with the centre on it removable?

After reading your message I went out and screwed down the end to the stub.It appears to only ratchet one way as it should.Must be pretty specialized.
